Now a global star, Donnie proposes to his girlfriend, Bianca Taylor, who accepts. When Bianca suggests relocating to Los Angeles to start a new life, Donnie hesitates, unwilling to leave Philadelphia and Rocky behind.
Meanwhile, Ivan Drago, the former Soviet boxer who killed Donnie’s father, Apollo Creed, in 1985, seeks to restore his family’s honor. With the help of promoter Buddy Marcelle, Ivan arranges a match between Donnie and his son, Viktor Drago. When Rocky refuses to support Donnie’s decision to accept the challenge, Donnie moves to Los Angeles.
In LA, Donnie and Bianca settle into a luxurious apartment near Mary Anne, Apollo’s widow and Donnie’s adoptive mother. As they adjust to their new life and prepare for the upcoming fight, Bianca discovers she is pregnant. Donnie enlists Tony “Little Duke” Evers, the son of his father’s former trainer, to help with his training. Overwhelmed by recent events, Donnie rushes into the match and suffers severe injuries. Viktor is disqualified for hitting Donnie while he is down, allowing Donnie to retain his title. However, Viktor gains immense popularity in Russia and secures a series of high-profile victories.
Physically and emotionally shattered, Donnie grows distant from Bianca. Mary Anne reaches out to Rocky, who reconciles with Donnie and agrees to train him for a rematch against Viktor. Meanwhile, Viktor endures grueling training under Ivan’s harsh methods, excelling in every challenge. Bianca gives birth to their daughter, Amara, with Rocky named her godfather. However, they soon realize Amara is deaf, inheriting the condition from Bianca’s progressive hearing disorder. As Viktor taunts Donnie publicly, he struggles under the relentless pressure from his father, who revels in the media spotlight and attention from Russian delegates.
