Storyline:
The infamous criminal Divine, living under the alias Babs Johnson, resides with her mentally unstable mother Edie, her delinquent son Crackers, and her traveling companion Cotton. They inhabit a trailer on the outskirts of Phoenix, Maryland, marked by a gazing ball and a pair of plastic pink flamingos. When Divine is declared “the filthiest person alive” by a tabloid, her rivals, Connie and Raymond Marble, plot to steal her title.
The Marbles operate a sinister black market baby ring. They abduct young women, have them impregnated by their servant Channing, and sell the babies to lesbian couples. The profits fund their pornography shops and a heroin distribution network targeting inner-city elementary schools. Raymond also engages in exhibitionism, using a sausage or turkey neck to shock women and steal their purses. However, a transgender individual foils his plan by exposing themselves, leaving Raymond stunned.
To gather intel on Divine, the Marbles employ a spy named Cookie, who seduces Crackers. In a notorious scene, Cookie and Crackers engage in intimate relations while crushing a live chicken between them, observed voyeuristically by Cotton. Cookie later reveals Divine’s identity, location, and family details, including her upcoming birthday party.
The Marbles send Divine a box of human feces as a birthday gift, accompanied by a mocking card declaring themselves “the filthiest people alive.” Fearing her title is at risk, Divine vows to eliminate whoever sent the package. Meanwhile, Channing, left alone, mimics his employers by dressing in Connie’s clothes and mocking their behavior. Upon discovering this, the Marbles fire Channing and lock him in a closet.
